Prepfresh, a takeaway at 15 Tarbock Road, Huyton, Knowsley was given the score after assessment on June 23, the Food Standards Agency's website shows.
It means that of Knowsley's 107 takeaways with ratings, 54 (50%) have ratings of five and just one has a zero rating.
