16 Swirls, a takeaway at 24 - 26 Mount Pleasant, Liverpool was given the score after assessment on July 1, the Food Standards Agency's website shows.
It means that of Liverpool's 553 takeaways with ratings, 227 (41%) have ratings of five and just six have zero ratings.
