Pizza Magic 2, a takeaway at 57 Market Street, Hoylake, Wirral was given the minimum score after assessment on May 23, the Food Standards Agency's website shows.
It means that of Wirral's 351 takeaways with ratings, 145 (41%) have ratings of five and just three have zero ratings.
