The kebab shop, a takeaway at 119 Windsor Street, Liverpool was given the score after assessment on July 7, the Food Standards Agency's website shows.
It means that of Liverpool's 556 takeaways with ratings, 229 (41%) have ratings of five and just five have zero ratings.
