Happy Valley, a takeaway at 204 Knowsley Road, Bootle was given the score after assessment on September 15, the Food Standards Agency's website shows.
It means that of Sefton's 269 takeaways with ratings, 190 (71%) have ratings of five and none have zero ratings.
