Piri Piri Kitchen, a takeaway at Unit 3 300, Cleveland Street, Birkenhead, Wirral was given the score after assessment on August 9, the Food Standards Agency's website shows.
It means that of Wirral's 348 takeaways with ratings, 144 (41%) have ratings of five and just two have zero ratings.
