Pizza Bar, a takeaway at 35 New Chester Road, New Ferry, Wirral was given the score after assessment on August 22, the Food Standards Agency's website shows.
It means that of Wirral's 349 takeaways with ratings, 145 (42%) have ratings of five and just two have zero ratings.
